This is the screen used to create or edit a patient Alert. See Concepts|Alerts for background.

The fields are as follows:
Date - the start date for the alert
End Date - the end date for the alert. This can be left blank if the alert has no end date.
Alert Type - select from those available - these are set via Administration|Types|Patient Alert Type
Priority - this displays the priority of the alert - see above link
Reason - a short version of the Notes text
Product - the product that created the alert, if any
Author - the user that created the alert
Clinician - the associated clinician
Notes - the long version of the reason for the alert
Status - can be either In Progress or Completed
