This simple form prints out the data you need when identifying a patient when performing an in-house IDEXX VetLab Station test.  It is arranged exactly as you see on the input screen.

Attach this form to your in-house investigation. When you bill an in-house test, the investigation will appear. Click ok then "Apply" in billed item.  Then click on Invoice item to open investigation window and click "Print Form".  Prints to an A4 page.
