Euthanasia Consent Form

This is a euthanasia consent form I've developed from our old "hand-filled-in" pre-OV era.

It's a Patient Form formatted to print on A4 paper; currently it has a left margin of 6.5cm to allow for use on our clinic letterhead, so you may wish to alter the left margin.

Please find attached a Vetnostic Pathology Submission Form.  This form will merge customer name /patient name, species, breed, age /clinic name & address/vet name /investigation number  - details for printing and sending to lab.

Can be used manually through patient documents or set up with Investigations.

Postcard Reminder

The attached open office template can be used for Postcard reminders.  Please modify text to suit you practice.

Note: OpenVPMS does not currently support multiple postcard per sheet formats.

In-house urinalysis record

This template can be linked to a urinalysis product and investigation type.

It can then be printed and used to record the results and then be scanned and linked back to the investigation request. It requires version 1.4 +

Version 2.0

Version 2.0 requires no printing. It can be previewed which produces a pdf form. The form can be completed using a pdf reader and then printed to a pdf to save on paper. It is important to note that saving the pdf after you fill in the form will not save the completed form- you need to print it to pdf.

Referral letter with an input field for notes

We use this template to send letters to a number of sources: other vets that send us cases, council reports, etc. Pretty much anything that requires a merged patient and customer detail.

The "To whom this letter is addressed" field lets you arbitarily type the recipients name.

The "Notes" field lets you type a big chunk of text and have it drop into the letter itself.

We use this template to create prescriptions for use with chemists.

Hint:

When adding a Patient letter, make sure you check you are the clinician before  you choose your template document or your name may not be merged.

Hospital Forms

This is our hospital forms that we print out for each patient. Obviously not suited to all clinics but just a few ideas on there.

Also some advice from any computer wiz would be great...

Before the last upgrade I was able to have procedure as an input field and have them copied throughout the document so once the document was previewed we were able to change the "procedure" once and it would automatically change them through the document.

Hospital Chart

This is a day based observation chart for hospitalised patients - using OpenOffice you can customise it to suit your hospital.

 

We print it double sided on our networked printer but we find that we have to print it from a "Print Preview" in order to print 2 sided. If we print from within Open VPMS it will only print one sided. We can't send the 2 sided request to the printer using the OP interface (printer driver limitation probably).

 

 

Patient Admission Label (7.2cm x 5.4cm)

We use this patient admission label to label xrays and possessions of patients that might be kept in our hospital while they stay.

- It has a large space on the top of the label to allow if to fit in our permanent radiograph labelling device.

- We print it on a Dymo label printer (not our favourite brand for various reasons - ask me in the forums :) )
