Hi Jim,

Typically these forms are contributed by someone who gets the original electronic form and adds the necessary merge fields etc and the adds the form to the shared resources area on the web site.  If you can get access to a word or open office template document of the form and post here I am sure someone will be able to add the necessary merge fields.  :-)

How does a NSW Central Animal Records microchip rego form differ from the CAR Standard Subscription Form (https://www.car.com.au/sites/default/files/CAR%20Standard%20subs.pdf)?

Apparently NSW Dept of Local Govt can't supply editable forms, only PDFs. Any way around this ?

It looks pretty easy to copy to me. If it's ok, I can easily make one this afternoon and post it on this forum. If it's not right just let me know.

I made a start on the form, but just need to confirm some things before going any further.

Does anyone have the current macros for:

Client surname (in patient document), Client first name (in patient document), Client title (in patient document), microchip number

The document I have copied, didn't have a place for filling in patient details (breed etc) or client address etc. Do you think there is another form or a page missing?

I was surprised how little info NSW wanted for their chip data as well when I did these forms: I meant to ask on the forum this very same question, but got tangled up in the 1.6betas and forgot about it.
